How do I schedule my first appointment?

Scheduling your first appointment is easy.  Contact us by calling us at 219-628-6463  


What are the office hours?

Dr. Sacks is in office Monday, Tuesday and Thursday from 10 a.m. until 7 p.m.

Dr. Samelson is in office Monday through Thursday 10 a.m. until 7 p.m. and Friday 10 a.m. until 3 p.m.

What if I do not have insurance?
We also accept cash and credit card for payment. 

What should I bring to my first appointment?
Please bring a copy of your insurance card and a credit or debit card to your first appointment and arrive at our office at least 10 minutes early to complete paperwork.

What if I have to cancel my appointment?
We understand that there may be times when you may need to cancel your appointment.  If you need to cancel and/or reschedule your appointment please call our office at least 24 hours before your scheduled appointment.  Cancellations made less then 24 hours may incur a cancellation fee.

Do you file claims with my insurance company?
As a courtesy to our clients we submit claims to the insurance company for reimbursement.  Co-payments are collected at the time of service.  You will be notified of and responsible for paying any amount not covered, including deductibles.  If we are not contracted with your specific insurance company ("Out of network") full payment will be required at the time of service.  A billing statement will be provided for you to file directly with your insurance company.

How much time will the psychologist spend with me (or my child) at the appointment?
Appointments are scheduled for 50 to 60 minutes.  At your first appointment we will get to know one another and discuss your concerns, pertinent medical history, and treatment options. 

What is the difference between a psychologist and a psychiatrist?
A psychiatrist may prescribe medication. 

Do you offer psychiatric services?
We do not currently offer psychiatric services.
